The survey signals an urgent call to government and relevant agencies to take action seriously, as Thailand has fully entered an aging society phase. If the welfare system remains outdated and fails to address the real-life needs of most elderly citizens, the resulting problems will have wide-ranging impacts on seniors, families, and the nation's long-term economic burden. Notably, elderly citizens are not merely demanding increased financial assistance; what they need most is convenient access to state services that accommodate age-related limitations—including local support staff, home-based services, targeted outreach, and simplified procedures. Many seniors lack genuine access to their own entitlements. The elderly also prioritize basic security essentials for living, including age-appropriate financial support, quality healthcare, and systematic long-term care, rather than ad-hoc emergency measures. Good welfare is not simply handing out money occasionally; it must build security across all life dimensions: health, income, housing, and care when dependent. An aging society is everyone's future concern, since all people will eventually grow old. Welfare investment is therefore not a budget burden but an investment in quality of life, reducing inequality, and strengthening social stability long-term. The government should use these survey findings as a guide to urgently reform the elderly welfare system to support the rapidly expanding aging population, designing welfare systems tailored to different elderly groups so all can access their rights equally, conveniently, and with dignity. The government must recognize that this should not be a temporary vote-catching policy followed by neglect, which would burden the country and citizens more than genuine and sustainable assistance.

Major ports and industries would inevitably generate substantial waste. Environmental conservation groups and southern civil society networks have mounted broad opposition campaigns. Recently, Prime Minister Anuthon Charanweerakul reaffirmed the project is not permanently abandoned but could be revived whenever deemed necessary and geopolitically suitable. However, Deputy PM and Finance Minister Ekniti Nitithamprasert, chairing the Land Bridge review, previously concluded the plan should be shelved due to poor returns, low feasibility, high budgetary risk, and serious threats to marine ecosystems, coastal livelihoods, food security, and forests. The committee ultimately determined the project has low commercial viability, carries significant environmental risk, and lacks social consensus. Although Anuthon says the plan remains open, no timeline or endpoint is clear. More pressing is an apparent lack of government mega-projects to drive transformative economic growth—Thailand has lacked major infrastructure initiatives for years, and current projects fall short of generating breakthrough economic stimulus.

A 65-year-old man presented to hospital in early July with over a month of continuous diarrhea, coming in only after finishing a construction job elsewhere. During the physical examination before ultrasound, emergency medicine physician Dr. Bei Qili immediately noticed alarming signs: the upper abdomen was visibly bulging and abnormally hard to palpation—findings that prompted deep concern even before imaging. Ultrasound confirmed that the liver was filled with tumor nodules.

History-taking revealed the patient was an HBV carrier but had never received regular monitoring. He admitted the abdominal swelling had been present for 1-2 months; he had only intended to visit the clinic during a break to collect medication, but the diagnostic results came as a shock.

Dr. Bei Qili explained that being an HBV carrier does not necessarily lead directly to cirrhosis. Most carriers, if the virus is in a low-activity phase and liver function remains consistently normal, can coexist with the virus throughout life and may not require treatment. However, key factors that worsen disease include ongoing viral activity, chronic liver inflammation, and additional risk factors such as advanced age, alcohol consumption, fatty liver, or diabetes.

Scientific studies show that among patients with chronic hepatitis B with active viral replication and inflammation, approximately 15-20% develop cirrhotic scarring within 15-20 years due to repeated liver damage and repair cycles, eventually progressing to end-stage cirrhosis.

Dr. Bei Qili warned that HBV carriers—even those without symptoms and with normal liver function on blood tests—must undergo regular monitoring. Most patients should visit a gastroenterology or hepatology clinic every 6-12 months. High-risk groups, such as those with cirrhosis, high viral load, family history of liver cancer, or those undergoing treatment, should be checked every 6 months or more frequently at the physician's discretion.

The doctor stressed that comprehensive monitoring is essential and should not be neglected. Beyond blood tests assessing liver function (ALT and AST) and alpha-fetoprotein (AFP), abdominal ultrasound is mandatory. Blood work evaluates inflammation, AFP is only a supporting marker, while abdominal ultrasound is the most important screening tool for cirrhosis, liver nodules, and early-stage hepatocellular carcinoma.
